How to Create a Consent Form
A Consent form ensures Patients are fully informed and voluntarily agree to Medical procedures or treatments, protecting Patient rights, facilitating clear communication, and providing legal documentation and protection for both Patients and Healthcare Providers.
- The contents of this user manual will consist of the following information:
- Consent Text
- Upload Attachments
- This user manual will start on the Diary screen.
- Click on the Settings button on the Navigation bar.
- The Settings screen will open.
- Click on the Entity Settings tab.
- The Entity Settings tab will open.
- Click on the Workflow Event Templates panel.
- The Workflow Event Templates panel will expand.
- Click on the Choose Event Type to Add Template for... drop-down menu.
- Select the Consent Form event.
- Click on the +Add Template button.
- The Consent Form Workflow Event Template screen will open.
- Click on the Template Description field to add a name for your new form.
- Click on the Placeholders drop-down menu to select the relevant placeholder to add.
- For more information on how to use placeholders, please refer to the user manual: Workflow Event (WFE) Placeholders.
- Click on the Workflow Event Class drop-down menu to select the desired option.
- Clinical: The Clinical Consent form will be found in the Patient file.
- Admin: The Admin Consent form will be found Admin Event History.
- Click on the Consent Form panel.
- The Consent Form panel will expand.
Consent Text
- Click on the Consent Text field to enter the desired information.
- Click on the Choose... drop-down menu to select a placeholder to add. For more information regarding what a placeholder is and an explanation of what information each placeholder will populate, please refer to the user manual: Workflow Event (WFE) Placeholders.

- After the placeholder is selected it will be added to the Text field.
Upload Attachments
- Click on the Choose File button to upload an attachment.
- Click on the Save button to save the contents of the Consent form that you have just set up.
- The Consent form that was set up will appear on the Workflow Event Template list.
Please Note: The Template has not been saved as of yet and has been highlighted in red.
- Click on the Save button to save all changes that have been made.
Please Note: Once the custom Consent form has been successfully saved it will no longer be highlighted in red.
