How to Use the Recurring Assessment Form



The Recurring Assessment form simplifies the documentation of examinations that occur at regular intervals or periodically over time. This feature supports the ongoing monitoring of changes, progress, or trends, aiding in diagnosis, and the development of treatment plans.

Please Note: In order to use the Recurring Assessment form, set up thereof is needed. For more information regarding the set-up of the Recurring Assessment form, please refer to the user manual: Workflow Event Template: Recurring Assessment Setup.

  • This user manual will consist of the following information:
    • Form Data

  • This user manual will start on the Diary screen.


  • Click on the desired Patient's booking.


  • The Diary sidebar will open.

  • Click on the Clinical button.


  • The Clinical screen will open.


  • Click on the Forms Library drop-down menu on the Clinical sidebar. 


  • Select the Recurring Assessment.


  • The Recurring Assessment will be added to the Clinical sidebar under the Active Forms section. 


  • The Recurring Assessment screen will open.


Please Note: The screen will open on the Template that has been saved as the Default Template when the set-up of the form was done. The data on the form is determined by the setup.


Form Data
The information that is needed to capture the details of the Recurring Assessment.



  • Hover over the Information icon to view additional details regarding the Recurring Assessment.



  • The information regarding each Recurring Assessment that needs to be performed on the Patient will also be displayed.



  • Click on the Load Previous button to load all the previous data that was captured for this specific Recurring Assessment Template.


  • The Previous data will be loaded onto the screen.


  • Click on the + button to add a new entry.


  • The Assessment Values screen will open.


Please Note: The Display Time will automatically be set to the current time.


    • Select the desired information on the screen to log the data of the Recurring Assessment.


    • Click on the Ok button to log the details. 


  • The New Entry will be logged.


Please Note: Once an Entry has been logged it cannot be removed. 

  • An indication will be given to advise when the Next Assessment is due at.
