How to Create a PDF Printover from a Custom Form
Generating a PDF Printover from a Custom form enables a Practitioner to electronically complete a form uploaded by the Patient, following instructions provided during the Custom form completion process.
Please Note: A Custom form will first need to be created. The Custom form must include the File Upload option, allowing Patients to upload a PDF file. After which you can convert the uploaded PDF into a PDF Printover that you can fill in. - This user manual will start on the Diary screen.
Send Custom Form to Patient
The form that you would like to be completed is sent to the Patient.- Click on the booking of the desired Patient.
- The Diary sidebar will open.
- Click on the drop-up menu on the myGC button.
- Click on the Send Custom Form button.

Patient Receives Custom Form
The form is received by the Patient via Email/SMS, allowing them to fill it out and submit it through the myGC platform.
Completed Custom Form is Received by the Practitioner
Once the Patient submits the completed form, the Practitioner will have access to the Patients' answers.- Click on the Patient booking.
- The Diary sidebar will open.
- Click on the Clinical button.
- The Clinical Case screen will open on the Custom form with all the Patients' answers.
- The Custom form that was completed by the Patient will be added to the Clinical sidebar under the Active Forms section.
PDF Printover is Created by the Practitioner
The PDF document that the Patient uploaded is used to Create a PDF Printover.- Click on the + Create PDF Printover button on the uploaded PDF.
- The PDF Printover screen will open.
- The PDF Printover will be added to the Clinical sidebar under the Active Forms section.
- The PDF Printover can now be used to fill in the Patient's document.
