Medical History Screen: Chronic: Navigation and Use

A Chronic Condition is an illness that lasts a long time or recurs frequently. A Chronic Condition is a health condition or disease that has long-term or persistent effects or a disease that develops over time. When a disease's course lasts more than three months, it is referred to as chronic. The purpose of capturing Chronic Conditions is to advise a Practitioner that the Patient has long-lasting illnesses and/or conditions.

  • The contents of this user manual will consist of the following information:
    • Chronic Conditions
    • Chronic Medication
  • This user manual will start on the Medical History screen, General tab.


  • Click/Tap on the Chronic tab.


    • The Chronic tab will open.



Chronic Conditions

A Chronic Condition is a health condition or disease that has long-term or persistent effects or a disease that develops over time. When a disease's course lasts more than three months, it is referred to as chronic.


  • Click/Tap on the Chronic Conditions panel.


    • Click/Tap on the Add Chronic Condition field to enter the relevant information.


      • An Add Note field will appear.


        • Click/Tap on the Add Note field to enter the relevant information.


  • Click/Tap on the Add Conditions button.


    • The Chronic Condition information will be added to the Chronic tab.


Please Note: More than one Chronic Condition entry can be added.


    Chronic Medication

    Chronic medication is prescribed for long-term management of ongoing health conditions like diabetes and hypertension, aimed at controlling symptoms and preventing disease progression.


    • Click/Tap on the Chronic Medication panel.


      • Click/Tap on the Add Medication field to enter the relevant information.


        • An Add Note field will appear.


          • Click/Tap on the Add Note field to enter the relevant information.


        • A Link to Diagnosis field will appear.


    Please Note: This field will only be available if a Chronic Condition has been added.

    • Click/Tap on the Add Medication button.


      • The Chronic Medication information will be added to the Chronic tab.


    • Click/Tap on the Save Chronic Conditions button to save any changes made.


      • Notification will appear indicating that the Chronic Condition was saved successfully.
