Medical History Screen: General: Navigation and Use
The General tab covers basic questions such as how much the Patient smokes, consumes alcohol or recreational drugs, as well as how much exercise is performed weekly by the Patient.
Please Note: The information filled in on this form will pull through to the Practitioner's site, giving instant access to the recorded information.

Smoking
The amount of cigarettes that you smoke a day.
- Click/Tap on the Cigarettes Per Day drop-down menu to Select the relevant option.
- Click/Tap on the Add Note About Smoking text field to enter the relevant information.
Alcohol Consumption
The amount of Alcohol you consume in the period of a week.
- Click/Tap on the Alcoholic Drinks Per Week drop-down menu to Select the relevant option.
- Click/Tap on the Add Note About Alcohol text field to enter the relevant information.
Exercise
The amount of exercise you take part in, in the span of a week.
- Click/Tap on the Average Hours Exercise Per Week drop-down menu to Select the relevant option.
- Click/Tap on the Add a Note About Exercise text field to enter the relevant information.
Recreational Drug Use
Any non-medical, drug-related substance that you have consumed, either currently or in the past.
- Click/Tap on the Select Recreational Drug Use Option drop-down menu to Select the relevant option.
- Click/Tap on the Add a Note About Recreational Drug Use text field to enter the relevant information.
General Notes
A free text field that can be used by the Patient to make any notes for future reference.
- Click/Tap on the Save General History button after all relevant changes have been made.
