Why Use This Function:

When a Patient's consent is required, the Practitioner is able to send an electronic Consent Form through the myGC App. This form can also be signed directly within the app, eliminating the need for printing and scanning documents and promoting a paperless Practice and process. Consent is a mutual agreement between two or more individuals, such as sharing sensitive information or performing a procedure. It covers both the Patient and Practitioner, ensuring both parties are aware of, and agree to the terms and conditions.

The Contents of This User Manual:

• Search 
• Filter
• Incomplete 
• Complete

• This user manual will start on the Health screen.

• For more information on how to navigate to the Health screen, please refer to the user manual: How to Navigate the Health Screen on myGC.

• Click on the Consent Forms button.
  

• The Consent Forms screen will open.
  

Search 

Allows you to search for a Consent Form from a specific Practitioner.

• Click on the Search Consent Forms field to enter the relevant details of the Consent Form you are searching for.

Filter

Filters enable you to refine the entries of the Consent Forms displayed on the screen.

Incomplete 

The Consent Forms that have not yet been actioned still need to be completed.

• Click on the Incomplete option.

• The list will be filtered to only show the Consent Forms that have not yet been completed.

• Click on the desired Consent Form to view the information.

• The Consent Form will expand.

• Click on the Download button to save a copy of the attachment to your device.

• Click on the Complete button to sign the form.

• The Consent Form screen will open.

Complete 

Consent Forms that have been actioned and signed.

• Click on the Complete option.

• The list will be filtered to only show the Consent Forms that have been completed.

• Click on the desired entry to view the contents thereof.

• The Consent Form will expand.

• Click on the Download button to save a copy of the attachment to your device.

• Click on the View Completed button to view the form.

• The Consent Form screen will open.

• Click on the Download button to save a copy of the attachment to your device.

• The information of the Signee will be displayed with a date and time stamp to advise by whom and when the Consent Form was signed. 

• Click on the Close button to close the Consent Form screen.
