Disposal - Discharge

The process of leaving a Hospital following treatment is known as Hospital discharge. When a Patient no longer requires inpatient treatment and are able to return home, a Hospital will discharge the Patient. Alternatively, the Patient can be discharged from a Hospital and transferred to another sort of Facility. When a Patient is Discharged, the Discharge section will be completed.

  • Click on the Disposal panel to expand it. 


  • The Disposal screen will open when the panel is expanded.


    • The Doctor will complete what happened to the Patient while they were in the ER. Were they admitted to the Hospital, Discharged, Transferred to another facility or Died. Under every option is a section that needs to be completed. 

Discharge

The process of leaving a Hospital following treatment is known as Hospital discharge. When a Patient no longer requires inpatient treatment and are able to return home, a Hospital will discharge the Patient. Alternatively, the Patient can be discharged from a Hospital and transferred to another sort of Facility. When a Patient is Discharged, the following section will be completed. 

  • Click on the Discharge button. 


  • The Discharge screen will open.


  • The following information needs to be completed in the Discharge section:


    • Click on the drop-down menu in the To follow up with field to select if the Patient needs to do a follow up visit with one of the specific Doctors from the options on the menu. 


      • ER: The ER is the Emergency Room. If the Patient needs to go back for a follow up visit with the Emergency Room, the ER option will be selected. 
      • GP: The GP is a General Practitioner. If the Patient needs to do a follow up visit with a General Practitioner, the GP option will be selected. 
      • Specialist: The Specialist is a Doctor who specialises in a specific Medical field. If the Patient needs to do a follow up visit with a Specialist, the Specialist option will be selected. The Specialist details will be captured further down in the Discharge section.
      • Allied: The Allied is Allied Workers. Allied health professionals are involved in the delivery of health or related services such as disease and disorder identification, evaluation, and prevention, nutritional and nutrition services, rehabilitation, and health system administration, to name a few. Dental hygienists, diagnostic medical sonographers, and dietitians are just a few examples of allied health professionals. If the Patient needs to do a follow up visit with an Allied worker, the Allied option will be selected. 
      • Other: The Other option can be any Other Doctor or Facility that is not mentioned in one of the above options. 


    • Click in the Follow up date field if the user needs to do a follow up visit with one of the Doctors as selected from the Follow up with option. 


      • The calendar date picker will open. Select a date on the calendar when the Patient must come back for the follow up visit.


      • The Follow up date will display as follow in the Follow up date field.


    • The Advice to patient is a Free text field. The Doctor can make any Note here for the Patient to give any advice for their treatment at home etc. 


  • From date: The From date is the start date. In this case, the From date is the start date of the Patient sick leave.
  • To date: The To date is the end date. In this case, the To date is the end date of the Patient sick leave and when they can return to work.
    • The Absent from Work date will default to the current / todays date when the Form is completed but can be changed manually with the date picker. The absent date will indicate the From and To date that the patient is booked off sick. Click in the From or To date field, the date picker will open and show the calendar, where the user can select the applicable From and To date.
    • The Light Duty date is the first day the patient needs to return to work for light duty tasks and will be used in circumstances where the patient is not booked off sick, but not able to perform their regular everyday tasks. When an employer is unable to perform their regular job obligations, light duty is used to describe work that is less physically or mentally demanding. Click in the From and To date field, the date picker will open and show the calendar, where the user can select the applicable From and To date.
    • The Normal Duty date is the date that the patient can start work again as normal after being booked off sick. Normal duty is when the Patient can return to their regular job responsibilities every day, full time again. Click in the From date field, the date picker will open and show the calendar, where the user can select the applicable From date.
Referral Details

  • The Referral details will be completed in the Referral section for when the Patient is referred to a Specialist for further treatment / diagnosis.


    • Speciality: The Speciality is that of the Referral Doctor. When the Patient is referred to a Specialist for further treatment / diagnosis the Speciality and Specialist details must be completed. The Speciality is a certain field in which a person decided to specialise in. You can select the Referring Doctors Speciality from the list. If the Doctor is searched and selected from the External (Medibase) Search, the Speciality will automatically pull through in this field.
    • Specialist Name: The Name and Surname of the Referring Specialist that the Patient is being referred to in this case. If the Doctor is searched and selected from the External (Medibase) Search, the Specialist Name and Surname will automatically pull through in this field.


    • Click on the Search External button. The Medibase Search screen will open where the user searches for a Specialist from the Medibase database.


      • The Search External option will allow the user to connect the Emergency Room to Medibase and handle all search operations within the EMR Form. The user can search the Specialist from the Medibase database and select the Specialist. The Speciality, Specialist Name and Surname and Contact Number will automatically pull through when this option is used. Medibase is the definitive source of healthcare contact information in GoodX. Medibase prides itself as the largest, most accurate, most complete and most up to date source of healthcare contact information available. The Online Directory is freely available to GoodX clients, for any and all healthcare information needs.


    • Click in the Contact Number text field to complete the Contact Number of the Specialist that is involved in this Hospital Case. If the Doctor is searched and selected from the External (Medibase) Search, the Specialist Contact Number will automatically pull through in this field.
    • Click in the Note to reference text field to complete the Note to reference. The Doctor can make any Note here about the Patient for the Referral Doctor.
Clinical Buttons

  • The following Clinical buttons can be used on the Discharge screen after the Discharge section was completed:

    • From the Discharge section, the user can click on the Create Referral Letter button to generate a Referral Letter for the Patient with the Referring Doctor / Specialist details. Click on the Referral Letter link or related button to open the manual for more information on how the Referral Letter works. 


    • From the Discharge section, the user can click on the Create Sick Note button to generate a Sick Note (Medical Certificate) for the Patient which they can send to their employer. Click on the Sick Note link or related button to open the manual for more information on how the Referral Letter works. 



    • The Clear button will discard all the details that were completed in the Discharge section. 

  • The Complete tab/s will turn green to indicate the section/s is completed.

  • Click on the Close button to exit the EMR screen without saving.

    • Click on the Save and Close drop-up arrow for more options: 

      • Save: Click on the Save button to save the information captured on the Form.
      • Save Lines to New Macro: Click on the  Save Lines To New Macro button to save the lines captured as a Macro.

    • Click on Save and Close to save the form or the changes made.
