Custom Form (Kiosk)

The Custom Form is an online document that has been set up by the Practitioner, using available templates, for the Patient to fill in prior to the start of an appointment. A Practitioner or Receptionist is able to send forms to a Kiosk, from a Patient's booking. These custom forms could consist of any information, such as a Covid 19 questionnaire, to a basic form with information that the Patient is required to know.
